Class Active Students
For class active students, academic leave of absence submission is registered through BAAK website. Pay attention to requirements acceptable to submit academic leave of absence from BAAK.
Non-class Students
For non-class active students, thus academic leave of absence submission must be registered from Psychology Study Program Secretary Office, acceptable by online through email or offline in Depok or Kalimalang office. Listed below are in need requirements for academic leave of absence:
- Grade summary downloaded from studentsite
- A copy or scanned of Family Card (Kartu Keluarga)
- A scanned of students’ ID Card
- Leave of absence submission form within acceptable format by Psychology Study Program Secretary Office
All requirements must be emailed to adm.sekjurpsiug@gmail.com with email subject of CUTIAKADEMIK_Nama_NPM. Assigned staff will give a follow-up letter for students’ leave of absence submission to BAAK.
Notable for students:
- Person whom signed the submission form must be parents or students’ guardian by which written in Family Card, and also students must sign above duty stamp
- Signature must be inked, not digital
